Procedure to Request Services
Students who wish to receive accommodations (academic adjustments and/or environmental modifications) are responsible for initiating the process with the Center for Disability Resources by identifying as a student with a disability, providing appropriate documentation, and requesting the relevant accommodations.
Students are responsible to request services and accommodations using the following procedure:
1. Students will identify as an individual with a disability by submitting appropriate documentation to support the request prior to the first semester of attending classes at Lebanon Valley College (LVC) or the first semester students are requesting accommodations.

2. Students will be notified of his or her eligibility for an intake interview. Students will make an appointment to meet with the Director of Disability Resources to review documentation and to determine appropriate accommodations. (In order to reactivate their status, re-enrolled students must meet with the Director of Disability Resources.)
3. During the meeting with the Director, students may request accommodations that are reasonable and appropriate to the course of study, supported by documentation, and in accordance with college policies. All accommodations must relate to a functional limitation described by the student and supported by documentation.
4. Determination of appropriate accommodations is a collaborative, interactive process between the student and the Director. The student and Director will sign an Accommodation Agreement, indicating the agreed upon accommodations. The accommodations will remain in effect; unless, the student and Director meet to review and possibly alter or add to the accommodations. Students will receive a copy of the Accommodation Agreement
5. Students will also be asked to review and sign a Consent to Release Information Form in which the student determines permission for the staff in the Center for Disability Resources to interact with professors, parents, and other relevant college personnel regarding the student's disability and accommodations, as necessary. This release covers only those issues related to the disability and accommodations.
6. Based on the Accommodation Agreement, the Director of Disability Resources will develop an Accommodation Letter for students to present to those professors from whom they will be requesting accommodations. After the first semester of receiving accommodations, students will be responsible for picking up the Accommodation Letter at the beginning of each semester.
7. Students are responsible for meeting with his or her professors, presenting the Accommodation Letter, and discussing the accommodations requested in a timely manner. Faculty members are not responsible for providing accommodations students have not requested or if students have not presented them with a letter. Accommodations are not retroactive. Students must self-advocate.
8. If a student encounters any problems or concerns regarding the approved accommodations, it is the student's responsibility to inform the Director of Disability Resources. The Director is available to provide assistance and support for resolving both student and faculty concerns.
